CDE Child Care Providers (Yellow Attendance Sheet)

CDE Payment Timelines & Document Guidelines

A yellow attendance sheet is used in CDE funded programs to document a child’s need for services.  CDE funded programs include CalWORKs Stage 2 and 3, Alternative Payment, and Family Child Care Home Education Network.  All the information on this form is pre-printed. Please do not alter any of the pre-printed information on the form.  If any of this information is incorrect, please contact CCRC.

An attendance sheet for each child will be mailed to you after care has been authorized.  From then on, CCRC will send you an attendance sheet by the 1st of the month.  If you do not receive an attendance sheet by the 1st week of the month, call Provider Payments at (818) 717-1000, ext. 4721 to request a reprint.

Payments are electronically transferred according to the date schedule below.  Payment should appear within 24 hours of the posted date.   Please check your provider pay card or bank account for availability of funds.

 New Timeline as of July 1, 2011

  • Attendance sheets received by the 4th business day of the month will have payment electronically transferred on the 14th business day of the same month.  Payment should appear within 24 hours.  Please check your card/bank for availability of funds.
  • Attendance sheets received from the 5th business day of the month to the 8th business day of the month will have payment electronically transferred on the last business day of the same month.  Payment should appear within 24 hours.  Please check your card/bank for availability of funds.
  • Attendance sheets received after the 8th business day of the month will have payments electronically transferred on the 14th business day of the next month.  Payment should appear within 24 hours.  Please check your card/bank for availability of fund.

Document Guidelines

  • In order for the Attendance Sheet to be processed, the following information must be properly completed.
  • The Attendance Sheet must be completed in blue or black ink; correction fluid/tape is not acceptable.
  • Attendance sheets must be completed on a daily basis with exact time (to the minute) by the appropriate person as he or she is dropping off and picking up the child from the child care provider’s care.
  • The child care provider must also complete the middle section when dropping off or picking up the child from school.
  • The enrolled parent and child care provider must sign and date the bottom of the attendance sheet declaring under penalty of perjury that the information reported is accurate.
  • Incomplete or inaccurately completed attendance sheets will have payment delayed until the information can be verified.

Submitting an Attendance Sheet

Original Attendance Sheets must be submitted to a CCRC office after the authorized period, otherwise payment may be delayed.

There are four methods for submitting an Attendance Sheet:

  • U.S. Mail
  • External Drop Box located outside the Antelope Valley or Chatsworth Office
  • Lobby Drop Box located inside the Antelope Valley or Chatsworth Office
  • Front Desk Representative
